Thank you for choosing Fawne for your permanent jewelry experience! Permanent jewelry involves welding a bracelet, anklet, or necklace directly onto you — no clasp, no seam — creating one continuous piece. This form explains how the process works and confirms your consent to receive the service.
Please read each section carefully and sign at the bottom. No services can be administered without a signed waiver.
Assumption of Risk
I understand that the Fawne Permanent Jewelry experience involves the use of a small welding device to fuse a chain directly onto my wrist, ankle, or neck, without the use of an open flame. I acknowledge that participation may involve inherent risks, including but not limited to brief heat sensation, minor burns, pinching, skin irritation, or other unforeseen reactions.
I voluntarily choose to participate and assume full responsibility for any risks, injuries, or damages that may occur as a result of my participation.
Release of Liability
I hereby release and hold harmless Fawne, its owners, employees, contractors, and affiliates from any and all claims, demands, damages, injuries, losses, or liabilities arising out of or related to my participation in the Permanent Jewelry experience, including but not limited to injuries caused by the welding equipment, materials, or the jewelry itself.
Material Information
The chains and jewelry used for this service are made of high-quality 14K Gold Filled and 925 Sterling Silver. Fawne may introduce additional metals or materials in the future, and I understand that this waiver applies to any metal or material used in my Permanent Jewelry service, whether or not it is specifically named above. I have been informed of the metals used via this waiver and understand that it is my responsibility to disclose any known metal allergies or sensitivities before the service begins.

I confirm that I do not have a pacemaker, a known allergy or sensitivity to metal, or any other medical condition that would be negatively impacted by wearing permanent jewelry, except as noted above. I understand that individuals with a pacemaker or conditions requiring frequent MRIs and CT scans should not undergo the welding procedure, and I agree to inform Fawne of any concerns before the service begins.
Jewelry Assembly & Wear Disclaimer
I understand that while permanent jewelry is designed to be long-lasting, delicate chains can still stretch, snag, or break with normal wear — particularly anklets and necklaces, which are more prone to catching on clothing, bags, or other objects. I understand it is my responsibility to be cautious during activities that could damage the jewelry (e.g., sports, heavy lifting, or exposure to harsh chemicals).
I understand that certain skin pH levels, sweat, or products such as lotions and hair products may cause Sterling Silver or Gold Filled jewelry to tarnish or discolor over time, and that this varies from person to person.
Aftercare, Removal & Re-Welds
I understand that permanent jewelry is intended to stay on and that removal requires cutting the chain with scissors or wire cutters. Fawne is not responsible for the cost of removal or any damage to the jewelry that occurs during removal.
I understand that Fawne offers a complimentary re-weld for jewelry applied by Fawne that breaks or comes undone within 30 days of my appointment, with proof of purchase. After 30 days, a flat re-weld fee of $30 applies, regardless of the piece. In the case of a Fawne permanent jewelry piece having to be cut or broken for the purpose of a medical procedure or surgery, Fawne will waive the re-weld fee for the first occurrence with proof of the piece having to be removed for medical reasoning.
Fawne does not guarantee the ability to re-weld chains that have been significantly damaged, pulled, or shortened. If staff deem the chain unable to be re-welded, then the customer may repurchase a permanent jewelry service.
I understand that Fawne guarantees the quality of the weld itself but does not guarantee the chain or jewelry against normal wear and tear, and that all permanent jewelry sales are final — no refunds or exchanges once the service is complete.
Cancellation & Rescheduling
Appointments are scheduled for 30 minutes and require a $15 non-refundable fee to book. I understand that if I am more than 15 minutes late to my appointment, Fawne may cancel my appointment and the $15 fee will not be refunded. I understand that if I no-show my appointment, the $15 fee will not be refunded.
I may reschedule my appointment with a minimum of 24 hours' notice. Once my service is completed, the $15 fee will be applied toward the total cost of my service.
I agree to abide by Fawne's cancellation and rescheduling policy. I understand that all waivers must be signed prior to my appointment, and that refusal to sign this waiver may result in my appointment being canceled. I understand that Fawne staff reserve the right to refuse service at any time, including for failure to act appropriately, arrival under the influence of substances, or creation of an unsafe environment.
Photography & Marketing (Optional)
I grant permission for Fawne to use photographs or videos taken during my Permanent Jewelry experience for marketing, social media, or promotional purposes.

Agreement & Electronic Signature
By signing below, I confirm that I have read, understood, and voluntarily agree to this waiver and release of liability, and to the “Permanent Jewelry Terms, Policies, and Aftercare” as posted on Fawne's website and/or provided to me at my appointment, market, or private event. I voluntarily consent to receive permanent jewelry from Fawne and assume all risks involved.
I agree that my typed or handwritten full name constitutes a legally binding electronic signature, and I voluntarily consent to be bound by the terms of this agreement.

Waiver for Minors (Clients Under 16)
For clients under the age of 16, a parent or legal guardian must be present for the entire appointment and must sign below to give consent. The minor must be able to sit completely still throughout the welding process.
I, the parent or legal guardian of the minor named below, give my full consent for the minor to receive permanent jewelry from Fawne. I confirm that I have read, understood, and agree to all terms outlined in this waiver on the minor's behalf, including the potential risks and responsibilities involved with permanent jewelry. I confirm that the minor does not have a pacemaker, a known metal allergy, or any medical condition that would be negatively impacted by wearing permanent jewelry, and I accept responsibility for overseeing the minor's aftercare.
I agree that my typed or handwritten full name constitutes a legally binding electronic signature, and I voluntarily consent to be bound by the terms of this agreement on behalf of the minor.
Minor's Photography Release (Optional)
I give Fawne permission to take photographs or video of the minor's jewelry and to use these for marketing purposes, including social media, websites, and promotional materials.
